At Forward Trust, we believe that everyone deserves the opportunity to build a healthier, more positive future. We're looking for a compassionate and motivated Health and Wellbeing Practitioner to join our integrated healthcare team, supporting adults affected by drug and alcohol use.

This is a rewarding opportunity to work with individuals on their recovery journey, providing person-centred interventions that empower people to achieve lasting change.

About the Role

As a Health and Wellbeing Practitioner, you'll manage your own caseload and deliver high-quality psychosocial interventions to adults experiencing substance misuse issues.

Working within a multidisciplinary team, you'll complete assessments, develop SMART care plans and deliver structured one-to-one and group interventions, using both harm reduction and recovery-focused approaches tailored to individual needs.

You'll work closely with healthcare professionals and partner agencies to ensure clients receive holistic support that addresses wider issues including housing, employment, benefits and family relationships.

Flexibility is essential within this role. This may include occasional evening or weekend working and, with reasonable notice, travelling to other services within your cluster to support service delivery and ensure continuity of care.
